Birth Certificates

The Highland County Health Department issues Birth Certificates for people born in Ohio (from 1909 onward). Each Birth Certificate costs $25.

There is a $2 fee for using a credit/debit card.

How to Get a Certified Copy:

  1. Online: www.vitalchek.com

  2. Mail: Print and complete the Certificate Request Form.

    • Mail the form, a letter of request, $25 per copy, and a self-addressed stamped envelope to:

  • Are you looking for a birth certificate for someone born before 1909? If so, you might be searching for a record that does not exist. Healthcare facilities and individuals born at home were not required to file an Ohio Birth certificate until December 1908.

  • Congratulations on your newborn! Birth certificates are usually ready for issue 1-2 weeks from the time the child was born.

  • Corrections to a birth certificate include fixing the spelling of a name (e.g., changing "Sarah" to "Sara") or adding a first, middle, or last name that was left out.

    This process cannot be used to change a legal name. For legal name changes, refer to the "Changes" section below.

  • All name change requests are processed through probate court. You can find more information on how to change your name by calling Highland County Probate Court at (937)393-9981.

  • For unmarried parents, a father can be added to a birth certificate with a Paternity Affidavit. This form can only be used if no father is currently listed on the birth certificate. You can find more information on establishing paternity on the Ohio Paternity Enhancement Program Website.

  • All adoption records are located at the Ohio Department of Health. There are different laws for viewing adoption records depending on what year the adoption took place. For additional information, please refer to the Ohio Department of Health Website.

Death Certificates

The Highland County Health Department issues Death Certificates for people whose death occurred in Highland County (from 1909 onward). Each Death Certificate costs $25.

  • Are you requesting a death certificate for someone born before 1909? If so, you might be searching for a record that does not exist. Healthcare facilities and funeral homes were not required to file a Ohio death certificate until 1909.

  • If a correction must be made to a death certificate after it has been filed, a Death Affidavit or Supplementary Medical Certification must be filed. Contact the funeral home that facilitated the burial or cremation to request the changes.

  • The family of a veteran is entitled to one free copy of that veteran’s death certificate. A VA application must be submitted by either the funeral home or the Veteran’s Administration Office.

  • In most cases, signed death certificates are available seven days after the date of death.
